Who are you covering?

Cover the ones who come running to you

Nobody plans to need ₦2 million on a Tuesday night.

Yet that's how hospital bills arrive in Nigeria: without warning, in full, and in cash. See what one bad year could cost your family, then pick the plan that turns it into a known yearly cost.

It starts with a phone call

At that moment you're not thinking about stocks or school fees. You just want this person to live.

So you do what Nigerians do: you call, you transfer, you borrow, you sell. The emergency is the illness. The disaster is the delay and the scramble for money while someone you love waits at the desk.

The usual system

Get sick first. Find money second.

Cover The Ones

A system set up before the emergency.

More than 70% of health spending in Nigeria is out-of-pocket (World Bank / WHO health expenditure data). About 13% of Nigerians had any health insurance in 2025.

Credit where it's due

State health insurance is a good start. Here's where it stops.

If your community already has a state scheme, keep it. It does real good. But the way state schemes are built creates the same limits everywhere — and families feel them at the worst moment.

Common design features in published state scheme guidelines we've reviewed. Schemes differ by state; this isn't a judgement on any single agency.

Mama Ada, 68 · state card only

Sunday 9:40pm. A stroke warning.

  1. Ward clinic closed. Headache, weak arm.
  2. Hired car, hours on the road to a referral hospital.
  3. “Deposit first” — the scan and admission are extra.
  4. The specialist is in another state. The card stops at the border ✕
Same night · with ClearlineHMO

A system, not a scramble.

  1. 24/7 doctor on the phone: “Go to hospital now.”
  2. The hospital she chose. She shows her ID card.
  3. CT scan and admission approved within plan limits.
  4. Specialist anywhere in Nigeria — same card ✓

Already on a state scheme? Keep it — and add the layer it's missing.

Let ClearlineHMO be the hospital, specialist and anywhere-in-Nigeria layer on top. A card is a promise. A system is the promise kept at 11:15pm.

The Health Budget Planner

Most families never add it up. In two minutes you'll see your expected yearly health spend, what a bad year looks like, how many months of income it would swallow, and exactly which plans would cover it and at what cost.

  • Family mode: adults, children, parents, pregnancy, chronic conditions, your city and your income.
  • Business mode: the hidden cost of absence, medical advances and exits, the premium, tax effect and ROI.
  • Your report: print it, save it or send it to yourself on WhatsApp.

Why Clearline

An HMO is only as good as the system behind the card.

Hospitals treat faster when they trust they'll be paid. That trust takes decades, not a sales pitch.

Since 1994

More than 30 years of paying hospitals. NHIA-accredited Clearline International Limited, Code 003, RC 493596.

₦1bn capital base

Two and a half times the NHIA requirement for a national HMO.

Nationwide network

2,109+ accredited hospitals and clinics on the August 2026 schedule; 2,000+ searchable here.

GreenLife pharmacy

Same group: long-term medicines and home delivery, so "the drug isn't available" happens less.

Clinix diagnostics

Same group: laboratory tests and imaging you can actually get done.

24/7 support

Telemedicine, the Clearline app, and the contact centre on 0700 245 245 245.

Azuka Orumgbe
Why Clearline through me

I'm Azuka. For more than 1,200 days I've repeated one message: don't let sickness become a money crisis.

I work in Corporate Partnerships at ClearlineHMO. I sit with HR managers, business owners, pastors, association leaders and families at home and abroad, and help them put cover in place before they need it. I've supported healthcare conversations with 30+ organisations.

I once lost a big contract to a cheaper HMO after six weeks of comparing benefit tables. When people can't see the difference, they choose the cheaper price. So I made the difference visible: in the playbook, in the planner, and on every call. Including when the cheaper plan is the right answer.

What I promise you

  • A reply the same working day, from me, not a bot
  • Your hospital route agreed before you pay
  • The limits and exclusions told to you plainly
  • If your card is refused at the desk, I'm on the phone and I follow up until it's resolved

What I won't promise

  • That every treatment is covered: limits and exclusions are real
  • Approval above your plan's limits
  • A price that isn't on the official rate sheet

Honesty now is cheaper than disappointment at 1:47am.

Questions

The things you should ask before paying

Which ClearlineHMO plan is right for me?

Start with who needs cover. For yourself or your family (ages 0–75) with no pre-existing condition: KiaKia (₦3,500, telemedicine), Clear Value (₦84,000, Band C–D hospitals) or Clear Advantage (₦250,000, Band B–D and a semi-private ward). For anyone already on medication for a condition, or a parent aged 50–75: Clear Elite (₦600,000, up to ₦3m a year), which covers pre-existing conditions from day one. For 11–29 staff: Bronze or Silver SME. For 30+ staff: the corporate tiers. From abroad: Diaspora Individual or Group. The Health Budget Planner on this site recommends a mix from your own numbers.

How much does health insurance cost in Nigeria with Clearline?

2026 prices per person per year: KiaKia ₦3,500; Clear Value ₦84,000; Clear Advantage ₦250,000; Clear Elite ₦600,000; Diaspora Individual ₦288,000; Diaspora Group from ₦93,870. SME: Bronze ₦84,000, Silver ₦108,000 per staff. Corporate: from ₦61,525 (Bronze) to ₦678,500 (Platinum Plus) per staff; Superb ₦40,000 for 500+ lives.

Can I cover my whole family?

Yes. On retail plans each person is priced separately and the maximum family size is six. You can add up to five family members in the online registration. If you're employed, ask your employer first; company family premiums often cost less per head.

I already have hypertension, diabetes or another condition. Can I get cover?

Yes. Clear Elite was created principally for people with pre-existing or chronic conditions, for anyone aged 0 to 75, subject to underwriting. Care for the condition you already have can start the day cover begins. On Clear Value and Clear Advantage, pre-existing conditions are not covered: if you are taking medication for, say, hypertension when you join, those drugs are not covered until one year has passed, although other care you did not have before, including normal hospital admission, is covered after the 14-day waiting period.

Can I join if I'm already pregnant?

A pregnancy that has already started when you buy is treated as pre-existing, so Clear Value and Clear Advantage will not cover it. The strongest maternity cover (₦200,000 to ₦650,000 including caesarean) is on company plans. If you or your spouse is employed, ask the employer first, and talk to Azuka before you pay so you know exactly what is and isn't covered.

Can I register my parents?

Yes. Clear Elite is the plan most families choose for a parent aged 50–75, and it covers conditions they already have (like high blood pressure) from day one, subject to underwriting: 24 specialist types, CT and MRI, private-ward admission up to 30 days, surgery up to ₦500,000 and a global limit up to ₦3,000,000 a year. Choose "A parent 50–75" in registration.

Bands are price levels, not quality grades

Providers sit in tariff bands A–D. "Band B–D" gives you a wider set of hospitals than "C–D". Check the band of the hospital you'd actually want.

Waiting periods are real

Clear Value and Advantage: 14-day waiting period; surgeries after 12 months; optical, dental and chronic medicines for new conditions after 3 months. Buy before you need it.

Mix and match

Companies can put executives on Platinum, core staff on Gold and field teams on Bronze, with everyone on KiaKia as a floor.

Illustrative estimates from your inputs and editable assumptions; not a quotation. Premiums are 2026 reference rates. "Bad year" adds one major event (surgery or long admission) to your expected year. Health spending above 10% of household income is widely used (for example in UN SDG 3.8.2) as the threshold for catastrophic health expenditure.

FAQ

Questions people ask before paying

Plain answers. If yours isn't here, ask Azuka on WhatsApp.

I live abroad. Can I pay for family in Nigeria?

Yes. Register as the sponsor, add the person's details in Nigeria, and pay into the official Clearline account. Diaspora Individual suits one person; Diaspora Group suits 11 or more people, such as an extended family or association. For a parent aged 50–75, compare Clear Elite.

Is health insurance compulsory for employers?

Under the National Health Insurance Authority Act 2022, health insurance is mandatory in Nigeria, and employers with five or more staff are expected to provide cover. SME plans suit 11–29 staff; corporate tiers start at 30 enrollees.

Are there waiting periods?

Yes. Clear Value and Clear Advantage have a 14-day waiting period; surgeries become accessible after 12 months; optical, dental and chronic medication for new conditions after 3 months; and drugs for a condition you already had are not covered until 12 months have passed. Clear Elite covers pre-existing conditions from the day cover begins; its other benefits follow its own schedule (admissions and minor surgery from quarter 1, dental and physiotherapy in quarter 2, eye care in quarter 3, intermediate surgery in quarter 4).

What is not covered?

Every plan has exclusions. On Clear Value and Clear Advantage these include pre-existing conditions (including a pregnancy already underway). Others include cosmetic surgery, home and domiciliary care, self-inflicted injuries, transplant surgery, obesity treatment and virility drugs, subject to the policy terms. Always read the official benefit schedule before you pay.

What does "band" mean for hospitals?

Clearline providers sit in tariff bands (A–D). A band is a price level, not a quality grade. Clear Value uses Band C–D; Clear Advantage uses Band B–D; higher corporate tiers reach further. Use the hospital finder to check the band of the hospital you'd actually want.

Is there anything for associations, churches or town unions?

Yes. Download the free ClearlineHMO Community Health Programme 2026 from the Free books page. It costs three options for 100 to 2,000 members, lists hospitals in Lagos, Abuja, every major city and rural towns, and includes a group registration form. Azuka then prepares a written costing for your group.

Can I find a hospital before I pay?

Yes. Search 2,000+ provider records by state, city, type and band on this site, get directions, and choose one as your preferred hospital in registration. Azuka confirms the hospital route before you pay.

How do I pay, and how do I know it's safe?

No payment is needed to register. After Azuka confirms your plan and hospital, pay only into the official company account: Polaris Bank, Clearline International Limited, 1771104431. Never pay into a personal account, and keep your receipt.

How soon does cover start?

Member IDs and policy documents usually follow within 5 working days of confirmed payment. Waiting periods then apply.

What happens at the hospital?

Show your member ID at the HMO desk. For anything beyond a basic consultation, the hospital requests an authorisation (PA) code from Clearline. If you're kept waiting, ask whether the request has been sent and call the contact centre on 0700 245 245 245. If you enrolled through Azuka, WhatsApp him too.

What if a hospital asks for a deposit?

For covered care, say: "I'm a Clearline member. Here's my ID. Please send the authorisation request now." Then call 0700 245 245 245. In a genuine emergency, life comes first: get the patient stabilised, then sort out the paperwork.

Can my company get a tax benefit?

Under the Nigeria Tax Act 2025, staff-welfare premiums are generally deductible expenses for companies that pay company income tax. At 30%, every ₦100 of premium can reduce tax by about ₦30. Small companies may be exempt. Confirm with your tax adviser.
